Customer Relief Program
This Rebate is supplied by Florida Home Improvement associates (FHA) at no charge to you. FHA has engaged Arlington Mutual LLC (AM) to administer the program on its behalf. This rebate is not an insurance policy. It does not fulfill the requirements of any financial responsibility laws, nor does it relieve you of any loan that you have agreed to pay.
Benefits
In the case of involuntary unemployment within twelve months of your contract, we will rebate $400 per month, for the amount of time that you are unemployed, not to exceed six months. You must qualify in accordance with the terms below and the maximum rebate will not exceed $2,400.
Qualifying for a rebate
You are entitled to a rebate if you are “Involuntarily Unemployed” if after 30 days from the funding date, you have suffered a total loss of wages for a period of at least 30 days as a result of loss of employment. You must prove that you have been employed, full time, for 120 days immediately before you became unemployed. You must also have been approved for payment of state unemployment benefits. To claim your rebate, log onto consumerreliefprogram.org and complete the Get Relief form or call 1 888 225-0332.
Exclusions
You do not qualify if you are Self Employed, employed by FHA or AM, employed by a member of your household, retired, become employed or if you became unemployed due to, resignation, disability, willful or criminal misconduct , were notified of termination prior to contract.
Personal Information
You agree to authorize Arlington Mutual LLC to collect and disclose information needed to process and evaluate the rebate request. Any misrepresentation will void rebate.
You may not assign or transfer this rebate.
This rebate is provided to you at no charge and is effective as of the funding of your contract (signing). The rebate is effective for 12 months after the effective date. All requests must be submitted and approved within these 12 months.
This offer is only good for sales after March 17, 2009. Important restrictions and limitations apply. This program is subject to change or end at any time.
